1. Log in to My Account with OnePass.

2. Click on Account Activities.

3. Click on View Service Agreement Forms and select the form that you would like to view.

When prompted, enter your password in the following format:​

For orders before 26 Oct 2023: 

<Identification Number><Birthday in DDMmmYYYY>​

E.g. If your birthday is 1 Jan 2015, your password would look like this: ​

S1234567T01Jan2015

For orders after 26 Oct 2023: 

<Identification Number (last 4 NRIC characters)><Birthday in DDMmmYYYY>​

 567T01Jan2015

Do note that the birth date has to be double digits and your password is case-sensitive e.g. the first letter of the month is in uppercase. ​
